While CKD-EPI 2021 is the international gold standard for kidney disease diagnosis and staging, the US FDA historically approved thousands of drug package inserts using the Cockcroft-Gault equation.
In elderly or low-weight patients, Cockcroft-Gault CrCl is often lower than eGFR. Clinicians should use the safer (lower) estimate to prevent toxic accumulation.
